Job Description Medical Social Worker (MSW) PRN Make a Difference Beyond the Clinical Visit A patient’s well-being depends on more than medical care. Your support can help patients and families access resources, navigate challenges, and remain safely at home. Elara Caring is hiring a compassionate PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities. This position gives you the independence to work directly with patients in their homes while remaining connected to an experienced interdisciplinary team. Why You’ll Love This Opportunity Build meaningful relationships with patients and families Provide personalized, one-on-one support Work independently in the community Collaborate with a supportive interdisciplinary team Help patients access essential services and resources Enjoy flexibility based on patient needs and visit availability Access continuing education and professional development opportunities Receive tuition reimbursement if eligible Participate in a 401(k) with company match Access medical, dental, vision, paid time off, and additional benefits based on eligibility What You’ll Do Assess patients’ psychosocial, emotional, financial, and environmental needs Complete initial social work evaluations and follow-up visits Provide counseling, education, advocacy, and emotional support Connect patients and caregivers with community programs and financial resources Identify barriers affecting patient safety, recovery, or independence Help patients and families navigate healthcare and community services Participate in developing and updating individualized Plans of Care Collaborate with physicians, nurses, therapists, and other care team members Support discharge planning and continuity of care Communicate concerns or changes affecting patient well-being Complete accurate and timely clinical documentation What You’ll Bring Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education Current Maine Social Worker license in good standing At least one year of social work experience in a healthcare setting Strong counseling, communication, documentation, and advocacy skills Knowledge of community programs and available patient resources Ability to work independently in patients’ homes Willingness to travel throughout the assigned service area with 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations Bring Your Expertise Where It Matters Most Use your social work experience to help patients overcome barriers, access essential resources, and remain safely at home. The base salary for this position is $70.00 to $80.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
