Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This full-time Medication Therapy Management (MTM) pharmacist role offers a 13-week contract with the potential for direct hire, serving a specialty MTM environment. The position supports onsite or remote work options, requiring a current Louisiana pharmacist license and licensure in at least five additional states from a designated list, including California, Indiana, and New Jersey, among others.

Key Responsibilities:

• Prepare and dispense prescriptions per physician orders, adhering to protocols and pharmacy policies.
• Accurately interpret and transcribe verbal and written prescriptions into patient medication profiles.
• Maintain complete and precise pharmacy patient medication records.
• Dispense pharmaceuticals, including controlled substances, with strict compliance to legal requirements.
• Monitor medication regimens for contraindications, interactions, allergies, and appropriate dosing.
• Identify and report adverse drug reactions promptly.
• Provide clinical consultations to healthcare practitioners, recommending cost-effective therapeutic alternatives.
• Document all clinical interventions thoroughly.
• Participate in quality improvement initiatives and medication use reviews.
• Supervise pharmacy support personnel, verify pharmacy technician activities, and assist with performance evaluations.
• Ensure timely pharmacy services through effective prioritization and independent work.
• Conduct medication storage inspections, replacing outdated or unusable drugs.
• Professionally handle patient and provider communications.

Qualifications and Desired Experience:

• Current valid Louisiana Pharmacist license.
• Licensure in a minimum of five states from the specified list; commitment to obtain licensure in at least 10 additional states within 18 months.
• Prior retail or hospital pharmacy experience.
• Strong knowledge of medication therapy management practices.
