• 2 days per week -- Monday & Tuesday preferred, 9 am - 5 pm
• 15 - 20 patients per day, increasing to 25 as clinic grows
• Ages 2+ patient population
• Active CAQH enrollment required
• Medicare/Medicaid enrollment required -- PTAN number needed
• Hospital privileges required
• Paid malpractice insurance; pre-paid travel and housing expenses
• Assignment details and time entry in online portal
