Member Engagement Specialist (CST) - Bilingual, Spanish (Fully Remote)

Value-based palliative care provider group transforming serious illness and end-of-life care. Interdisciplinary teams reduce avoidable hospitalizations and improve quality of life. The Role: Conduct telephonic outreach to schedule clinical assessments and educate members on services. Provide ongoing customer service to members and caregivers and maintain enrollment targets.
