Title: Physical Therapist (PT)
Employment Type: Permanent | Full -Time
Location: Vaux Hall, NJ 07088

Schedule: 8:30 AM – 4:30 PM

Compensation: Hourly Rate: $55/hour (Based on experience) + Benefits

Patient Population

• Post-Acute Care
• Orthopedic Rehabilitation
• Neurological Rehabilitation
• Medically Complex Conditions
• Long-Term Care (LTC)

-> Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver evidence-based physical therapy services to improve mobility and functional independence.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Ensure compliance with Medicare guidelines, state regulations, and facility policies.

-> Qualifications

• Active New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Post-Acute, Orthopedic, or Neurological Rehabilitation preferred.
• Strong communication and documentation skills.
• CPR/BLS certification preferred.
