RN Case Manager

INTERIM HOME HEALTH

Interim HealthCare is seeking an RN Case Manager to coordinate and deliver patient-centered home health services. In this role, you’ll complete in-home nursing assessments, develop and manage individualized care plans, and collaborate with an interdisciplinary team to support optimal outcomes. You’ll educate patients and families, ensure high-quality, compassionate care, and maintain accurate clinical documentation.
