RN Case Manager (PRN)

Job Location Hillsboro, OH 45133 Overview PRN Case Manager works on an as needed basis and coordinates the care and service of selected patient populations across the continuum. Works collaboratively with interdisciplinary team both internal and external to the organization to improve patient care through effective utilization and monitoring of health care resources and assumes a leadership role to achieve desired clinical, financial and resource outcomes. Responsibilities Coordinate care and services for selected patient populations on an as needed basis. Collaborate with interdisciplinary team both internal and external to the organization. Improve patient care through effective utilization and monitoring of health care resources. Assume leadership role to achieve desired clinical, financial and resource outcomes. Qualifications Degree in Nursing and current RN license in the state of Ohio with at least three years of practice experience in Nursing. Excellent written and verbal communication skills. Well-rounded critical thinking skills. Creative problem solving skills. Proficient organization and planning. Ability to tolerate frequent interruptions and demanding workload. Knowledge of funding resources, services, clinical standards and outcomes. Knowledge of Medicare, Medicaid, Social Security and other entitlement programs. Time management skills and ability to work under pressure. Leadership skills and ability to use a wide range of interpersonal techniques and intervention modalities.
