RN Case ManagerSuncoast Hospice of Hillsborough, a member of Empath Health, is seeking an RN Case Manager to join the Empath team. Monday - Friday, daytime hours. Coverage area includes Hillsborough County, providing direct patient care in private homes and facilities.Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.What you'll do:Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.Following a patient's death, assists with closure contact and support in collaboration with IDT.Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician.
