Work Environment
This role is primarily remote, allowing for flexibility in work hours while maintaining a structured schedule. Full-time working hours are Monday to Friday from 8:00 AM to 4:30 PM, with potential for occasional evening or weekend commitments depending on client needs. Successful candidates will also have the flexibility to negotiate part-time hours based on specific client requirements. An essential part of this role includes being on call to address urgent client situations outside regular working hours.
Key Responsibilities
- Conduct in-depth assessments of seniors' psychosocial needs, taking into account emotional, social, and mental health factors to create comprehensive care plans.
- Collaborate closely with clients and their families to craft personalized care strategies that align with their unique goals and preferences.
- Provide counseling services and emotional support designed to help seniors navigate significant life transitions and associated challenges.
- Act as a staunch advocate for clients, ensuring their rights are upheld and that they have access to necessary community resources and services.
- Engage with a multidisciplinary team of healthcare providers, including nurses and caregivers, to coordinate care and monitor client progress effectively.
- Regularly visit clients to assess their living conditions, monitor their overall well-being, and address potential safety concerns.
- Facilitate open communication among family members involved in care, helping to resolve conflicts and foster effective collaboration.
- Educate clients and their families about community resources, support groups, and social programs that can help improve their quality of life.
- Maintain meticulous documentation of client assessments, care plans, progress notes, and significant interactions in compliance with organizational standards.
- Stay updated on current social work practices, policies, and ethical standards to ensure best practices in care delivery.

Qualifications
- A Master’s degree in Social Work from an accredited institution, reflecting a solid theoretical and practical grounding in the field.
- Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee, demonstrating a recognized level of expertise and compliance.
- A minimum of two years of relevant experience in social work, ideally within a community center or geriatric care environment.
- In-depth knowledge of social work principles, age-related issues, and the available community resources pertinent to seniors.
- Exceptional assessment and counseling abilities, coupled with strong problem-solving skills.
- Adept at establishing rapport, demonstrating empathy, and showcasing compassion towards clients and their families.
- Strong communication skills with a focus on effective collaboration across multidisciplinary teams.
- Proficiency in electronic health record (EHR) systems and relevant software applications.
Preferred Experience
Experience working within community support organizations and familiarity with geriatric populations are strongly preferred. Compensation and Benefits
The compensation for this full-time position ranges from USD 60,000 to 100,000 per year, commensurate with experience and qualifications.
