Role Description

This is a full-time, on-site role in North Coumty San Diego for a Hospice Liaison. The Hospice Liaison will work with healthcare providers to establish and maintain relationships, educate the community about hospice services, and help identify patients who may benefit from hospice care. Day-to-day tasks include representing the organization in the community, fostering relationships with referral sources, providing information about hospice and palliative care services, and collaborating with the care team to ensure the best possible patient outcomes.

Compensation
80-120 k

• Expertise in Hospice Care, End-of-Life Care, and Palliative Care
• Sales experience and ability to build strong partnerships with referral sources
• Knowledge and experience in Home Care and the broader healthcare system
• Outstanding communication and interpersonal skills
• Ability to work effectively in a team-centric, compassionate environment
• Proven organizational and time-management skills
• Experience in community outreach and education is a plus
• Current licensure as a healthcare professional is preferred but not required
• minimum 2 year experience in Hospice Marketing/sale
