As Circadia's full-time field home health NP, you'll see patients directly in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions—reducing preventable hospitalizations. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting. What You'll Do Direct patient care Conduct initial in-home assessments on patients Initiate/adjust plans of care Coordinate with referring and primary care providers Tech to bedside integration Introduce the Circadia Device during in-home assessment and help set-up in a 2 minute process. Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits Close the loop Complete documentation/charts within the same day. Home-based visit management Efficiently complete 6+ in-home visits per day across designated home health patients in the SoCal/OC area, focused on new admissions Quality & product feedback Surface field insights to improve workflows and tech integration in a home health setting Support NP onboarding, share best practices, and partner with clinical ops/product teams Regional flexibility Cover sites within a 30 mile radius; for sites outside of that radius, mileage and lodging covered per policy. How Success is Measured (KPIs) Visits: Target 6+ patient home visits/day Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter. Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time Must-Have Qualifications Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute Eligible for Medicare participation; company bills on your behalf 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently Valid CA driver's license; reliable transportation for in-home visits across the SoCal/OC area Comfortable conducting face-to-face visits in a home environment, including with patients requiring complex care Thrives in a dynamic, evolving startup environment Nice-to-Haves Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools Spanish preferred Mission + autonomy: Lead a high-impact program that advances proactive home-based care Tools that help: AI documentation, in-app training, integrated workflows; company laptop + mobile tools #J-18808-Ljbffr
