Job Summary The Cardiac Rehab Exercise Physiologist reports directly to program Manager, in conjunction with Subject Matter Experts. Key responsibilities include providing age‑appropriate, patient‑specific care that addresses the biological, emotional, psychological, physical fitness, and educational needs of the patients/family; collaborating with system care partners such as health psychology, diabetes education, and other wellness programs to ensure patients have access to all areas in the continuum of care. Through partnering with the multidisciplinary team of professionals, the development of individualized goal‑oriented care plans and exercise prescriptions places the Cardiac Rehab Exercise Physiologist at the centre of each patient’s success. Core Responsibilities and Essential Functions Patient Care: Assess patient risk factors that are modifiable and provide strategies to aid patient in successful therapeutic lifestyle changes. Participate in initial assessment and orientation of new participants; assist in development of plan of care with other staff members. Develop and adjust individual exercise prescription in accordance with daily patient responses to exercise according to ACSM and AACVPR guidelines. Supervise exercise session, monitor participants for inappropriate responses to exercise. Measure blood pressure, observe telemetry responses and blood glucose measurement per department policy. Document daily responses in Scott Care and EPIC. Maintain strict confidentiality concerning participants’ and families’ medical and personal information. Teaching/Education: Assess patient’s knowledge of cardiovascular disease, nutrition, risk factors and develop educational plan to meet individual needs. Lead patient education in 1:1 and/or group setting. Lead warm‑up, cool‑down, and strength training exercises. Team Responsibilities: Participate in emergency training sessions and assist as directed in emergency situations. Attend a minimum of scheduled staff meetings; if unable to attend, obtain information from meeting minutes or another team member. Support and assist in development of performance and process improvement initiatives designed to promote highest quality patient care. Maintain flexibility to respond to changing needs of the team. Contribute to cohesive group relations and communicate effectively with team members about individual patient findings or concerns about program issues. Technology: Basic working knowledge of Microsoft Outlook, Microsoft Word, Success Factors, Scott Care system and Epic EMR system. Self‑Development: Stay current in the field of cardiac rehabilitation and cardiovascular disease through journals, in‑services, webcasts, computer‑based learning modules, meetings, peer‑to‑peer teaching and conferences. Identify self‑learning needs and seek opportunities for improvement. Participate in group activities, meetings, in‑services, etc., as well as personal efforts to gain proficiency in the AACVPR Core Competencies. Required Minimum Education Bachelor's Degree in Exercise Science or related field. Required Minimum License(s) and Certification(s) Basic Life Support (BLS) through American Heart Association (AHA) – Required upon hire. Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) – Required within 180 days. Required Minimum Experience Experience exercising special populations including cardiac, geriatric, and diabetic patients. Knowledge of EKG, CAD, blood pressure and blood glucose monitoring and treatment options. Cardiac Rehab experience – Preferred. Group exercise leadership and public speaking experience – Preferred. Required Minimum Skills Knowledge of coronary heart disease, cardiopulmonary anatomy and physiology, electrocardiography, and exercising special cardiac populations.
