Clinical Consultant Social Worker (MSW/LSW)

Job Description Summary This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at assigned hospital. It is responsible for providing services to patients, families and significant others, including completion of biopsychosocial assessments, implementation of treatment plans and discharge planning to support or enhance biopsychosocial functioning. Responsibilities and Duties 30% Clinical Assessment – Conducts patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information pertaining to patient/client biopsychosocial functioning. Assessment includes developmental and age‑specific needs, ability to care for self and dependents, basic custodial and financial needs, meaningful social and emotional relationships, and comprehension and compliance with treatment recommendations. Develops care plan and discharge plan, and participates in identification of abuse, neglect or exploitation cases, referring them to legally mandated authorities. Provides crisis intervention, information and referral, and linkage to community resources. 30% Intervention Management – Develops a plan of intervention based on assessment to resolve identified problems and support biopsychosocial functioning. Applies individual, family or group modalities and clinical intervention skills such as treatment planning, continuum of care planning, supportive counseling, crisis intervention, problem‑oriented planning, and educational counseling. 25% Population Management – Coordinates clinical and financial outcomes of an assigned patient population. Works with interdisciplinary staff and external organizations to achieve timely, cost‑effective management of care. Monitors quality and service type delivered, liaisons with payers to manage post‑discharge finances, and ensures completion of treatment plans and reports. 10% Documentation and Regulatory Compliance – Thoroughly documents assessments and interventions in the patient’s medical record and information systems, complying with internal and external documentation requirements and regulatory standards. 5% Community Linkage – Maintains awareness of current hospital and community services and appropriate care options that assist patient functioning. Provides information to and facilitates successful linkages between those services and the patient/client. Minimum Qualifications Master's Degree in Social Work (required) Licensed Social Worker (LSW) license in the State of Ohio 1–2 years experience as an LSW, preferably in a patient‑centric setting Knowledge of medical terminology and computer skills DL‑HC Driver’s License if employed in a Home Care department (optional) Associates hired after 01/10/2019 must meet current qualifications; those with a B.S. who were hired prior to 01/10/2019 are grandfathered.
