Responsibilities Act as the primary point of contact for patients, addressing inquiries and providing support. Coordinate patient appointments and follow-up consultations to ensure continuity of care. Educate patients on monitoring services and equipment usage for optimal health outcomes. Build and maintain strong relationships with healthcare providers and community organizations. Document patient interactions and maintain accurate records in compliance with HIPAA regulations. Collaborate with internal teams to enhance patient care processes and service delivery. Conduct outreach initiatives to engage and inform potential patients about services offered. Stay updated on industry trends and best practices to continually improve patient experiences. Requirements Proven experience in patient care or healthcare liaison roles; Cardiology is a plus. Strong communication and interpersonal skills to foster relationships. Knowledge of Cardiology and patient care needs. Ability to work independently and as part of a collaborative team. Proficiency in using electronic health records and patient management systems. Compassionate demeanor with a genuine interest in patient welfare.
