OhioRISE Care Coordinator
The OhioRISE Care Coordinator provides family-centered, community-based care coordination and high-fidelity wraparound services to children and families with complex behavioral health and system-of-care needs.
In this role, you will partner with children, families, and their support networks to identify strengths and needs, develop individualized care plans, coordinate services, and build a team of providers and natural supports around each child. Care Coordinators develop crisis and transition plans, conduct ongoing assessments, and help families successfully navigate multiple systems of care.
This position is ideal for a compassionate, culturally responsive professional who enjoys building relationships, coordinating complex services, and helping children and families access the resources they need to thrive.
Key Responsibilities
Family-Centered Care Coordination
- Deliver structured service planning and care coordination using high-fidelity wraparound models.
- Partner with children and families to identify strengths, needs, goals, and natural supports.
- Develop, implement, and regularly update individualized, family-centered care plans.
- Develop crisis safety plans and transition plans with families and their support teams.
- Conduct comprehensive assessments, including the Child and Adolescent Needs and Strengths (CANS) assessment.
- Facilitate Child and Family Team Meetings and ensure services remain aligned with established care plans.
- Support children and families as they transition between systems and levels of care.
Community Collaboration & Resource Navigation
- Connect families with community resources, natural supports, and appropriate services.
- Maintain working knowledge of local systems, including health care, behavioral health, education, juvenile justice, and other community resources.
- Develop and coordinate wraparound teams based on each child's individual strengths and needs.
- Collaborate with families, providers, schools, community agencies, and other system partners to promote continuity of care.
- Advocate for children and families while promoting culturally responsive and strengths-based services.
Caseload & Documentation Management
- Manage assigned caseloads, with ICC caseloads of up to 10 and MCC caseloads of up to 25.
- Conduct required initial face-to-face visits within established timeframes.
- Prepare meeting agendas, document meeting notes, and maintain timely and accurate records.
- Complete required documentation in compliance with agency, regulatory, and program standards.
- Ensure timely delivery of services and meet established caseload and productivity requirements.
- Maintain fiscal and administrative responsibilities associated with assigned services.
Professional Development & Quality
- Participate in required training and ongoing professional development.
- Maintain CANS certification and required competencies.
- Participate in quality improvement and process improvement initiatives.
- Stay current on best practices related to children and families with complex behavioral health needs.
- Demonstrate cultural responsiveness and a commitment to serving diverse populations.
- Perform other duties as assigned.
Qualifications
- Bachelor's degree in Social Work, Counseling, Psychology, or a related field from an accredited college or university.
- Minimum of three years of experience working with children and families.
- Experience working with children and families experiencing serious emotional disturbance, trauma, co-occurring mental health and substance use disorders, and/or involvement with multiple systems of care.
- Strong understanding of complex family and community systems.
- Culturally responsive approach with experience managing complex cases.
- Strong communication, organization, problem-solving, and relationship-building skills.
- Ability to work independently while collaborating effectively with families, providers, and multidisciplinary teams.
- Proficiency with Microsoft Windows and standard Microsoft Office applications.
- Valid driver's license and ability to safely operate a motor vehicle in changing weather and community conditions.
Compensation & Benefits
Pay: Starting at $45,000/year, based on qualifications and experience.
