Community Case Manager: Discharge & Resource Linkage

Acenda Integrated Health is seeking a Case Manager to collaborate with clients, community services, and clinicians to ensure effective care plans. The role connects clients to housing, substance use services, mental health care, and primary care, with on-call support for crises. Base pay starts at $20.19/hour plus a $1,000 sign-on bonus.
