Travel is part of the job — this is a five-county footprint across Maryland’s Eastern Shore. A reliable vehicle and valid driver’s license are required. Strong organization is essential — you’ll be tracking multiple clients, deadlines, consents, and authorizations across several sites without daily oversight. Technology comfort is essential — you’ll be working in an electronic health record, conducting some sessions by secure video, and using electronic consent and authorization workflows. Independent, not isolated — you’ll have a supervisor and a team of case managers to coordinate with, but the day-to-day pace is yours to manage. Position Summary The Targeted Case Management Community Support Specialist conducts comprehensive TCM assessments and develops individualized care plans for new referrals and required six-month reassessments. This role focuses on assessment, treatment planning, and the documentation, consents, and authorizations that keep our clients’ services active and compliant. You will partner with our case managers, supervisors, and referral sources to identify each client’s needs and translate those needs into a clear, achievable plan. You will own the timelines for your own assessments and reassessments, the authorizations tied to them, and the consents and releases that make coordination of care possible. This is a contractual position with flexible scheduling. You decide when the work happens — within the deadlines set by Maryland Medicaid, our Administrative Service Organization (ASO), and program requirements. Travel across our Eastern Shore clinics and into clients’ homes and communities is a regular part of the role. Key Responsibilities Assessment and Care Planning Conduct comprehensive TCM assessments for new referrals within 20 days of authorization. Complete six-month reassessments and updated care plans within required timelines. Evaluate each client’s strengths, needs, functional impairments, behavioral health concerns, and service needs. Develop individualized care plans with measurable goals, target dates, proposed interventions, and clearly assigned responsibilities. Incorporate input from the client, family or other supports (when authorized), and other treatment providers. Provide clinical recommendations and referrals based on each client’s needs and eligibility. Authorizations — and Staying on Top of Them Submit initial authorization requests to the ASO accurately and promptly after intake. Track each client’s authorization status, units used, and renewal dates so coverage never lapses. Submit reauthorizations and uninsured benefit renewals (typically every 90 days for uninsured clients) before they expire. Communicate authorization status to case managers and supervisors proactively — flag issues early, not after they’ve become problems. Maintain documentation that supports each authorization in case of audit. Consents, Releases, and Confidentiality Obtain signed Agreements for Services, Consents to Treat, and Releases of Information at intake. Update consents and releases annually, or earlier when circumstances change. Document client confidentiality protections in accordance with HIPAA, 42 CFR Part 2 (when applicable), and Maryland state law. Track release expirations and revocations so the team always knows who we can and cannot speak with. Documentation and Compliance Maintain accurate, timely, and confidential documentation of every assessment, reassessment, and care plan. Document in the agency’s electronic health record (EHR) — including assessments, care plans, signatures, contact notes related to assessment, and uploaded supporting documents. Submit completed documentation within designated timeframes; do not let assessments or care plans sit past required deadlines. Ensure all work complies with COMAR 10.09.45, Maryland Medicaid requirements, BHA standards, and CBH internal policy. Be audit‑ready at all times. Documentation should be able to stand on its own, without needing context to explain it. Collaboration and Communication Communicate assessment findings and recommendations with the assigned case manager and supervisor. Coordinate with referral sources (hospitals, primary care, courts, other agencies) when appropriate and authorized. Participate in interdisciplinary team meetings as needed to support continuity of care. Respond to colleagues, supervisors, and referral sources in a timely, professional manner. Qualifications Education Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health field — required. Licensure (Required) Active Maryland clinical license in good standing, one of the following: LCSW-C (Licensed Certified Social Worker – Clinical) LCPC (Licensed Clinical Professional Counselor) LMSW (Licensed Master Social Worker) — accepted with appropriate clinical supervision per agency policy LGPC (Licensed Graduate Professional Counselor) — accepted with appropriate clinical supervision per agency policy Experience Experience completing behavioral health assessments, treatment planning, or TCM documentation — preferred. Experience working with adults with mental health, substance use, or co-occurring disorders — preferred. Familiarity with Maryland Medicaid and TCM standards (COMAR 10.09.45) — preferred. What You Need to Be Good At Clinical skill matters, but the candidates who thrive in this role share a specific set of strengths: Organization and Follow-Through Managing your own caseload across multiple clinic sites and home/community settings. Tracking dozens of deadlines simultaneously — assessments, reassessments, authorizations, consents — without needing reminders from a supervisor. Knowing where every client’s paperwork stands at any given moment. Building your own calendar, your own workflow, and your own system for staying current. Technology Fluency Comfortable working in an EHR — documenting, uploading, e‑signing, and navigating client charts efficiently. Comfortable conducting some assessments by secure video when clinically appropriate. Comfortable using electronic consent and authorization tools. Quick to learn new software, portals, and workflows. Independent Work Self‑directed and accountable — flexible scheduling means you decide when to work, but the deadlines are fixed. Honest about workload and timing. If a deadline is going to slip, you communicate before it slips, not after. Clinical Writing and Communication Strong clinical assessment and documentation skills — clear, specific, and compliant. Excellent written and verbal communication, both with clients and with the care team. Ability to translate clinical observations into goals and interventions clients can understand. Work Environment Travel This position requires regular travel across CBH’s Eastern Shore footprint. Sites and counties served include: Cambridge — Dorchester County Denton — Caroline County Easton — Talbot County Centreville — Queen Anne’s County Chestertown — Kent County Assessments and reassessments take place in CBH clinics, in clients’ homes, and in community settings. A reliable personal vehicle, valid Maryland driver’s license, and current auto insurance are required. Mileage reimbursement is provided in accordance with agency policy. Schedule This is a contractual role with flexible scheduling. You set your own hours within the deadlines defined by Maryland Medicaid, the ASO, our supervisor, and the program. Evening or weekend work is at your discretion. There is no required clock‑in time and no minimum daily presence in any single clinic location — but every assessment, care plan, consent, and authorization must be completed on time. Compensation Flat rate of $48 per completed note. This is a fee‑for‑service contractual position; the rate is all‑inclusive and covers travel, the visit, the authorization, and the documentation.
