This nurse coordinates patient assessments, develops and updates individualized care plans, administers medications and treatments, and closely monitors patient responses, collaborating with nephrologists, hospitalists, dietitians, pharmacists, and other members of the interdisciplinary team to ensure safe, evidence-based care. The nurse contributes to staffing coordination, workflow optimization, and effective communication during shift handoffs and multidisciplinary rounds.
