RN Case Manager

The RN Case Manager coordinates care for hospitalized patients with complex medical and social needs, with a strong focus on assessment, discharge planning, care coordination, resource utilization, and safe transitions of care.

Position Duties:

• Coordinate patient care and discharge planning
• Complete medical, functional, and social assessments
• Develop individualized plans of care
• Identify and address barriers to safe, timely discharge
• Coordinate community and post-acute resources
• Collaborate with physicians, nursing, patients, families, and interdisciplinary teams
• Review admissions, continued stays, and appropriate level of care
• Support appropriate resource utilization and length-of-stay management
• Participate in interdisciplinary rounds and case conferences
• Advocate for patients and support quality outcomes
• Provide education on case management and discharge-planning processes

Minimum Required Qualifications:

• Active Montana RN or Compact License
• BSN
• Three (3) years of experience as a Registered Nurse is required, acute care setting required.

• Knowledge of Medical Terminology.
• Knowledge of Desk Top software including Power Point and Excel, computer skills for data entry.

Preferred Qualifications:

• Strong discharge planning and care coordination experience
• ACM or CCM preferred, but not required

Length of Assignment: 13 weeks Shift / Hours per week: 5 days/week 8:00a-4:30p (40hrs) Start Date 10/5/2026
