In-Home Traveler Physician (Full-Time) - Connecticut State and Surrounding Markets

Signify Health, part of CVS Health®, is seeking a Full‑time Physician to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The role involves traveling up to 100% of the time across a defined coverage area, which may require overnight hotel stays, car rentals and/or flights. Holding multiple state licenses will be required, as dictated by business needs. Job Highlights The visit lasts about 45 minutes, allowing time to connect one‑on‑one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (diabetic eye exams, spirometry, etc.). You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen. In the Traveler Role, You Will: Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role Be flexible to travel locally and within licensed states, up to 100% of the time Receive additional per‑completion incentive pay for all in‑home visits Conduct virtual visits as needed Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (spirometry, DEE, etc.) A Note on Our Work Environment As an in‑home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in such settings. The number of evaluations completed per day may vary. Pay is dependent on experience, education, geography and other relevant criteria. The position is eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Services (DPS), as ordered, in addition to the base pay listed above.
