Memory Care SpecialistSedgewood Commons is a Specialized Memory Support Community offering individualized care for those living with Alzheimer's and other forms of dementia. Sedgewood Commons offers Skilled Nursing and Long-Term Care Services, as well as access to a variety of therapeutic and leisure activities. We incorporate past and current hobbies and learning opportunities into our daily therapy treatment plans, including art therapy, music therapy, and gardening. Each member of our team is provided onsite training in the use of standardized assessments including the BCAT, GDS, ACL, as well as training in evidenced based practice with support from researchers such as Allen, Reisburg, and Mansbach. We provide person-centered care approach, through our signature program-Compass. Individualized resident plans of care are developed to maximize function and engagement through environmental accommodations and task modifications. Care partners and families are integral members of the team, partnering with rehabilitation clinicians to sustain quality of life post discharge from therapy.
