This role includes following each patient's individualized Plan of Care, delivering treatments, and collaborating with the care team to support hospice patients, provide comfort, and enhance overall quality of life.If you're a compassionate LPN who values meaningful patient connections and autonomy in your practice, we'd love to meet you.QualificationsLPN Requirements & Core Responsibilities:Holds current Licensed Practical Nurse (LPN) license in state(s) of practice.Graduate of an accredited practical nurse or vocational nursing program required.Ability to pass background, consumer report and drug test.Provide direct patient care, follow the Plan of Care, and support end-of-life needs.Document all care accurately and promptly while educating patients and caregivers.Coordinate care by communicating patient needs to the interdisciplinary team.Manage pain and symptoms, use equipment safely, and maintain full HIPAA compliance.Must have reliable transportation, valid driver's license, and proof of auto insuranceLPN Benefits & Schedule:Schedule: M-F 8:00am to 5:00pm, weekend visit coverage as needed, full-time position.Health, dental, vision, and life insurance.Company-paid short/long-term disability and basic life insurance.401(k) with up to 4% company match.Tuition reimbursement and professional development opportunitiesPaid time off and paid holidays
