Want to practice OT the way it was meant to be done? Stern At Home Therapy is hiring Occupational Therapists to provide one-on-one care to patients in their homes throughout New Rochelle, NY. No productivity quotas, no double-booking, no clinic burnout — just meaningful 1:1 care with full clinical autonomy and a compensation structure built around your goals. Full-time 1099 therapists can earn up to $145,000 annually based on visit volume.

We've built our model around what clinicians actually need — flexible scheduling, tight administrative support, and multiple ways to structure your role. Whether you want a few PRN visits a week or a full-time income with W-2 benefits, we have an option that fits. Most therapists are seeing patients within two weeks of joining.

What the Role Looks Like:

• Evaluate and treat a primarily geriatric patient population in their homes throughout New Rochelle.
• Develop and manage individualized plans of care with full clinical autonomy.
• Help patients improve independence, safety, mobility, upper extremity function, and ADL performance.
• 1:1 care every visit — no double-booking, ever.
• Complete timely clinical documentation with admin team support for scheduling and logistics.
• Manage your caseload based on your availability and chosen employment structure.

Compensation:

• PRN: $72 per follow-up visit | $85 per evaluation.
• Part-Time W-2: $65 per follow-up visit | $75 per evaluation.
• Full-Time W-2 Tiered: $55–$70 per follow-up visit (increases with volume) | $75 per evaluation.
• Full-Time 1099 Tiered: $70–$80 per follow-up visit (increases with volume) | $85 per evaluation — earn up to $145,000 annually.
• Full-Time Salary: $92,000–$110,000 annually.

What You'll Need:

• Active NY Occupational Therapist license.
• Reliable transportation.
• Ability to work independently in a home-based setting.
• Strong clinical, communication, and time-management skills.
• Home health experience preferred but not required.
