We are seeking a Nurse Practitioner - Hospice - Pallative Care to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team.

Details:

Title: Nurse Practitioner - Hospice - Pallative Care

Type: Full time Permanent

Facility Location: 1900 S Main St Findlay, OH 45840

Shift:  First shift, Monday–Friday, 8:00 AM to 5:00 PM

Compensation and Benefits:

·         Competitive base salary based on experience and training (to be discussed at interview).

·         Travel and reliable personal transportation required (no vehicle provided by the hospital)

Responsibilities:

·         Diagnose and treat acute, episodic or chronic illnesses

·         Prescribe all necessary medications and treatments

·         Perform comprehensive physical examinations of patients

·         Develop and implement patient management policies and procedures

·         Facilitate referrals to other healthcare professionals and medical facilities

·         Communicate with collaborating physician or specialist regarding patient care

Qualifications:

·         Board Certified NP -  REQUIRED

·         OH license (or willing to obtain) -  REQUIRED

·         Experience as advanced provider  in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) -  REQUIRED

·         Clean malpractice and license -  REQUIRED

·         Date of agency's last communication with provider (put answer in "Additional Submission Details") -  REQUIRED
