Trauma-Informed Youth Case Manager

I Am Next, Inc. is seeking a Case Manager – Support Services to provide trauma‑informed support to transition‑aged youth with histories of foster care and housing instability. You will help develop individualized service plans, connect participants to housing, transportation, education, employment, and healthcare, and empower clients toward independence. You will work collaboratively to identify goals, track progress, and ensure compassionate, strengths‑based care using HMIS and other case #J-18808-Ljbffr I Am Next, Inc.
