•Service Setting: Outpatient Clinic (FQHC)
•Coverage Type: Clinical Only
•Coverage Period: April 20, 2026 - Ongoing (6 months)
•Clinical Shift Schedule: Tuesday - Friday, 8:00 AM - 6:00 PM (1-hour lunch)

PATIENT INFORMATION
•Patient Demographics: All ages
•Patient Volume/Census: Approximately 16 patients per day
•Case Mix: General optometry including eye exams, diagnosis of vision issues, prescribing corrective lenses, and treatment of eye conditions

FACILITY INFORMATION
•EMR System: Athena
•Office Equipment Available: Standard optometry equipment
•Support Staff Available: 1 Optometric Technician, 1 Optician
•Reason for Coverage: Supplemental Coverage

PRIVILEGES & COMPLIANCE
•Credentialing Timeline: Minimal (FQHC)

REQUIRED PROCEDURES
•Comprehensive eye exams
•Vision testing and correction
•Diagnosis and treatment of eye conditions
•Prescription of corrective lenses and therapeutic medications

JOB REQUIREMENTS
•Licenses: Active state license preferred; willing to license
•Board: National Board of Examiners in Optometry certification required
•Other Qualifications: Must be comfortable with independent charting

DUTIES & RESPONSIBILITIES
•Provide comprehensive eye care services
•Diagnose and treat visual and ocular conditions
•Prescribe corrective lenses and medications
•Collaborate with clinic staff
•Maintain accurate documentation in Athena EMR
