Hospice Registered Nurse Case ManagerA leading regional healthcare provider known for patient-centric care is seeking a Hospice Registered Nurse Case Manager to join its interdisciplinary hospice team. The organization is a respected community hospice that serves patients and families in homes, assisted living communities, and skilled nursing facilities. Full-time and per diem schedules are available with a steady weekday routine and a shared on-call rotation. This role suits an RN who brings calm clinical judgment, strong symptom management skills, and genuine compassion to end-of-life care. The hospice team works closely together, with physicians, social workers, chaplains, aides, and volunteers all supporting each patient and family. New nurses complete a structured orientation that includes shadow visits and documentation training, and leadership keeps caseloads and on-call expectations sustainable so nurses can give each family the time and attention they deserve.Core ResponsibilitiesManage a caseload of hospice patients across home and facility settingsComplete admission, routine, and recertification visits and comprehensive assessmentsDevelop and update individualized plans of care with the interdisciplinary teamProvide expert pain and symptom management in partnership with the hospice physicianEducate patients and families about the dying process, medications, and comfort measuresCoordinate care with aides, social workers, chaplains, and volunteersDocument visits accurately and on time in the electronic medical recordParticipate in weekly interdisciplinary group meetingsTake part in the on-call rotation and respond to urgent needsSupport families with dignity and compassion through end-of-life careProvide guidance and support to families at the time of death and coordinate post-death visitsMentor hospice aides and verify that aide care plans are followedRequired QualificationsActive, unrestricted RN license in the state of practice or an eligible compact licenseCurrent BLS or CPR certificationOne year or more of RN experience; hospice, home health, oncology, or palliative care experience preferredValid driver license, reliable transportation, and current auto insuranceStrong communication skills and emotional steadiness in sensitive situationsAbility to work independently and manage time across a territoryKnowledge of hospice regulations and Medicare hospice benefit preferredCommitment to patient dignity, privacy, and family-centered careBenefits & PayPay: $36.00 - $44.00 per hour plus on-call pay, based on experience and schedule.
