THE GROWTH YOU HELP PROVIDE AS A HOME HEALTH REGISTERED NURSE What You'll Do: Perform in‑home skilled nursing visits – SOCs, recerts, routine skilled visits, discharges; complete thorough assessments, implement plan of care, and educate patients/families. Document accurately and on time in the EMR (assessments, care provided, orders, signatures) to support billing, compliance, and continuity. Coordinate care & communicate with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current. Monitor progress & escape concerns (changes in condition, safety risks, non‑compliance) to prevent adverse events and avoid unnecessary hospitalizations. Support compliance & quality – follow policies, payer requirements, and home health standards; participate in QA and care plan updates.

We're looking for a nursing professional who can meet the following qualifications: 3+ years’ nursing experience Active Florida RN license Reliable transportation for in‑home visits Preferred: Prior home health/field experience SOC/Recert/Discharge experience Strong documentation & OASIS Spanish speaking
