Shift: Monday-Friday 0800-1630 (40 hours weekly, 36 guaranteed) Job Responsibilities:
RN case management
All home health visit types Required Qualifications:
Associates Degree
RN State of Ohio
Home health experience Additional Information:
Schedule details: 8 hours/day, 40 hours/week (36 guaranteed)
Productivity expectations: 30 units
On Call/Weekend Requirement: 1 week/weekend monthly
Coverage Areas: Lake, Ashtabula, Geauga County
Mileage reimbursement: Yes (between visits)
Nice to have: Home Care Home Base experience
Dress code: Blue/Black
