Job Title: Home Health Speech-Language Pathologist (SLP)
Location: Mount Angel, OR & Surrounding Areas
Start Date: ASAP
Shift: 3x8 Hour Days (Wednesday–Friday, 8:30 AM – 5:00 PM)
Schedule Note: 4x10s or 3x12s schedules are not available

Role Overview

We are seeking a dedicated Home Health Speech-Language Pathologist (SLP) to provide one-on-one patient care in a home health setting. This role focuses on delivering high-quality speech therapy services, managing patient caseloads efficiently, and ensuring accurate documentation using standardized systems.

Key Responsibilities

Provide speech-language therapy services to patients in home health settings
Manage a caseload of up to 6 patients per day (productivity standard: 30 points)
Perform patient assessments, develop care plans, and track progress
Complete accurate and timely documentation using required systems
Travel to patient homes and healthcare facilities within the assigned region
Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
Maintain compliance with healthcare regulations and organizational standards

Required Qualifications

Minimum 1 year of skilled home health experience
Active SLP licensure (or pending license accepted)
BLS (Basic Life Support) certification required
Experience with OASIS documentation ( required )
Valid driver’s license and reliable transportation
COVID-19 vaccination (primary series + one booster required)

Preferred Qualifications

Experience with EPIC charting system
Strong organizational and time-management skills
Ability to work independently in a home health environment

Work Environment & Additional Details

Home Health setting
Business casual attire or scrubs permitted
No weekend or on-call requirements
No floating between units
Block scheduling not available
Local candidates accepted (within 50–75 miles)

Travel & Coverage

Travel required within assigned territory (typically optimized for minimal drive time)
Occasional travel radius may extend up to 50+ miles
Mileage reimbursement provided at standard rate

Additional Notes

First-time travelers are welcome
Candidates must be comfortable working independently and managing schedules efficiently
