Your Role

• Provide comprehensive,relationship-driven care to Medicare Advantage patients
• Serve as both a clinician and trustedhealth coach , guiding behavior change and preventive care
• Maintain full accountability forpatient outcomes, experience, and total cost of care
• Conduct frequent touchpoints withpatients, including same-day and walk-in availability
• Coordinate care acrossspecialists, hospitals, and post-acute settings
• Lead and collaborate closely withyour care team to ensure seamless patient support
• Supervise and mentor AdvancedPractice Providers (APPs)

What We're Looking For

• MD or DO
• Board Certified/Eligible inInternal Medicine, Family Medicine, or Geriatrics
• Active or eligible Texas medicallicense
• Active DEA
• Passion for serving seniors andunderserved communities
• Commitment to a value-based,outcomes-driven model of care
• Strong interpersonal skills and arelationship-first mindset

Compensation & Benefits

• Salary: $250,000 - $306,714 (based onexperience)
