HHA - Smiling Heart (1265)Smiling Heart Home Health Services - Fort Lauderdale, FL 33309Job ResponsibilitiesDuties of the Home Health Aide (HHA) include, but are not limited to:Provides assistance with personal care, hygiene and activities of daily living.Encourages client participation in activities to the extent to which the client is ablePerforms duties of a Home Health Aide (HHA) as per: Scope of Practice and Florida LawAssists client to: a.)bed, b.)commode, c.)chair, and assist with ambulationTurns and positions bed bound clientsMeasures and records intake/output, as assignedMeasures and records temperature, pulse and respiration on each visitChanges bed linen if neededPrepares simple meals following dietary instructions as instructedMaintains a neat and clean environmentMay grocery shop one time a week for a list of ten items or less as neededInforms supervisor of any changes in client's condition or home situationFollows care plan as writtenProvides documentation of care given on Agency approved formsReports any changes in patient's condition, living conditions etc., to DON/Supervisor as they occurPerforms any other task/duty that is specifically assigned by supervisor, and for which Home Health Aide (HHA) has been specifically trained. Documentation of specific training must be included in employee's personnel fileConducts self in professional manner at all times and in all situationsProvides Agency with: required certifications, required license, necessary information to be able to verify experience, documentation of CEUsAccepts only those assignments for which he/she is qualified as per Florida LawComplies with all Agency policies and proceduresCommunicates with Agency about any problems or concernsComplies with HIPPA Privacy Rules, Policies and ProceduresReports any suspected violations of Privacy Practice to Privacy Official as soon as breach/possible breach is identified.QualificationsJob QualificationsMust provide evidence of formal trainingMust provide physical Home Health Aide (HHA) certificateMust provide documentation of clearance to work (green card, employment authorization card, social security, etc...)Must provide evidence of competency training and evaluation as well as evidence of at least annual attendance at in-service education programsMust be able to use Electronic Visit Verification (EVV) program on own cellular/ mobile deviceMust be able to provide own transportationMinimum of one (1) year current experience is required
