Home Care Coordinator Assistant

Job Summary Provides administrative support to the Home Care Coordinator and delivers personal care, homemaker services, personal interaction and restorative care to participants in their homes to maintain a healthy and independent lifestyle in the community. The personal care assistant works independently in a safe and courteous manner, under the direction of a care plan developed by the home care coordinator and participant, to provide care needed, allowing the maximum level of independence in the home. Job Duties Provides administrative support to the Home Care Coordinator and directs participant care including but not limited to: Personal care, nutrition and hydration, obtain vital signs Assist with ambulation and transfers of participant, active and passive range of motion Assist with prompting for medication compliance and care of participant incontinence Participates with indirect participant care responsibilities including but not limited to: Light housekeeping, assist with laundry, use supplies accurately and economically, and delivery of medication and supplies as needed May require on-call coverage for after-hours care for emergencies in home. Remains attentive to participant and personal care assistant safety responsibilities including appropriate use of good body mechanics, gait belt, back brace for transfers and ambulation of participants. Collects data for mandatory requirements and various quality improvement projects and is actively involved in action plans and implementation to improve quality for the LIFE Geisinger program. Completes accurate and thorough documentation for services provided according to program policy. Explains actions to participant before starting task, demonstrates that all care is done with full regard to participant rights, provides orientation and redirection, promotes independence by offering choices and fostering self-help skills, and communicates respect and caring verbally and non-verbally with participant and other staff. Works cooperatively with other members of the interdisciplinary team to provide quality of care to LIFE Geisinger participants. Qualifications & Education High School Diploma or Equivalent (GED) – required Certification(s) and License(s) – required (details provided upon hire) Experience – relevant experience in home care or related field preferred Skills & Competencies Strong communication skills Ability to work independently and as part of a team Good body mechanics and safety awareness Comfortable with routine data collection and documentation Demonstrated respect for participant rights and independence Benefits Healthcare benefits for full-time and part-time positions from day one, including vision and dental coverage.
