While the majority of the work will be conducted remotely, you will be part of a dedicated team that prioritizes face-to-face interactions during team meetings and case conferences to discuss patient treatment plans and progress.

Key Responsibilities\n

• Conduct comprehensive mental health evaluations, integrating medical history to assess the overall mental health and wellbeing of each patient.

• Design and implement individualized treatment plans that incorporate both psychiatric and medical needs, monitoring progress and adjusting strategies accordingly.

• Facilitate individual and group therapy sessions utilizing evidence-based practices, focusing on tailored interventions to foster personal growth and resilience.

• Engage in collaborative care with medical providers to ensure a seamless treatment experience for patients, discussing treatment progress and co-developing care strategies.

• Deliver crisis intervention support during mental health emergencies, providing immediate assistance and resources to clients in distress.

• Educate patients on the interrelation between mental and physical health and guide them toward effective self-management strategies.
\n
• Maintain meticulous and confidential documentation in alignment with legal standards and ethical practices, ensuring patient records are up-to-date and accessible for interdisciplinary reviews.

• Actively participate in team meetings and case conferences, contributing insights and fostering a culture of collaboration and support.

• Stay informed about advancements in mental health research and integrated care methodologies to continually enhance the quality of care provided.

Patient Population / Client Focus\n

This position focuses on a diverse client population, primarily consisting of individuals grappling with co-occurring psychiatric disorders and medical conditions. Our therapists are dedicated to addressing the unique interplay of these issues, fostering healing through integrated care approaches tailored to individual needs.

Qualifications\n

• Master's or doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in the state of New York.

• Strong foundation in understanding mental health challenges, particularly as they relate to patients with complex medical backgrounds.
\n
• Exceptional interpersonal skills, enabling effective communication and relationship-building with patients and colleagues alike.

• Familiarity with electronic health record systems and an aptitude for managing documentation efficiently.
