Patient Care Manager Fall RiverSchedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.Key ResponsibilitiesProvide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirementsCoordinate and monitor patient care deliverySupervise clinical staff to ensure consistent, appropriate serviceMaintain clear communication with patients, caregivers, referral sources, and field and office staffDrive quality outcomes by meeting department goals and supporting improvement initiativesModel professionalism and organizational values in daily workUphold compliance through required training, accurate reporting, and cooperation with auditsPromote a safe work environment by following established safety protocolsSupport staff growth through coaching and continuing educationPatient Care Manager QualificationsRegistered nurse with current licensure to practice nursing in the practicing state.Previous experience in home care setting with two years management or supervisory experience, preferred.Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
