About the Role

As a Physical Therapist at Concept Rehab, you will provide evidence-based evaluation and treatment to patients recovering from illness, injury, or surgery in a post-acute rehabilitation setting . You'll collaborate closely with interdisciplinary teams, patients, and families to help individuals regain mobility, strength, and independence.

This role is ideal for PTs who value clinical quality, teamwork, and professional development .

Key Responsibilities

• Evaluate patients within 24-48 hours of physician orders
• Develop and implement individualized treatment plans
• Provide skilled, patient-centered therapy interventions
• Supervise and collaborate with Physical Therapy Assistants
• Communicate effectively with physicians, nursing, families, and care teams
• Participate in interdisciplinary and care planning meetings
• Document evaluations, treatments, and progress accurately and timely
• Support discharge planning and caregiver education
• Participate in staff education and in-service training as appropriate

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy
• Active Iowa Physical Therapist license
• Strong communication and collaboration skills
