Medicaid Care Coordinator — Field-Travel Health Advocate

Humana

Humana is seeking a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care. You will assess members, develop personalized care plans, and connect families with resources to improve health outcomes. The role requires 75–90% local travel and field visits, with a schedule of Monday–Friday, 8am–5pm CST. Hybrid/remote options may apply, with travel as needed.
