We are seeking a highly motivated and experienced Director of Social Work to lead our social work team at our state-of-the-art facility in Lake Katrine, New York. As a key member of our leadership team, you will play a critical role in promoting the overall well-being and recovery of our patients, while fostering a collaborative and supportive work environment for our staff.

About the Role:

As the Director of Social Work, you will be responsible for providing strategic leadership and oversight to our social work department, ensuring that our patients receive comprehensive and individualized services to address their social, emotional, and psychological needs. You will work closely with our interdisciplinary treatment team to develop and implement patient-centered care plans, provide counseling and therapy services, and facilitate group activities and social events.

Responsibilities:

Lead, support, and supervise the Social Work team.

Provide case management and social work services for an assigned caseload of approximately 40 residents.

Welcome new residents and families and assist with the transition into the facility.

Assess and address residents' psychosocial, emotional, and quality-of-life needs.

Advocate for resident rights, preferences, and individual needs.

Participate in care conferences and interdisciplinary care planning.

Support residents and families with discharge planning, community resources, hospice services, and other needed services.

Oversee and follow up on resident and family grievances and concerns.

Collaborate closely with Nursing, Admissions, Activities, Administration, Medical Providers, and other departments.

Assist with admissions, readmissions, room changes, transfers, and discharges.

Ensure Social Work documentation, policies, and practices meet facility and regulatory requirements.

Provide staff education related to Resident Rights and other Social Work topics.

Participate in department leadership meetings and rotating on-call coverage.

Requirements:

Minimum BS/BA Degree in Social Work or related field. Masters preferred

Minimum 3 years experience in Social Work
