Bilingual LTSS Service Coordinator|Field-Based Case Manager

Elevance Health is seeking a LTSS Service Coordinator (Bilingual Spanish/English) to manage a defined caseload with a focus on comprehensive person-centered planning and inter-disciplinary care coordination in Hillsborough County, FL. You will assess needs, coordinate LTSS/IDD and BH/PH services, and document goals within PCSP while collaborating with the member's support circle.
