Physical Therapist The role focuses on evaluating patients' physical capabilities and designing individualized rehabilitation plans that enhance mobility, strength, and independence. Key activities include collaboration with multidisciplinary healthcare teams, education of patients and families on exercise routines and injury prevention, ongoing monitoring of progress through documentation, and adjustment of treatment strategies as necessary. The therapist also implements interventions across various settings and participates in discharge planning and follow‑up care. Role and Responsibilities Assess patients’ physical abilities and devise personalized treatment plans. Collaborate with health‑care teams to ensure comprehensive care. Educate patients and families on exercises and injury prevention. Monitor and document progress. Adjust treatments as needed based on patient response. Implement interventions across various settings. Participate in discharge planning and follow‑up care. Qualifications and Skills Doctorate in Physical Therapy. Active PT license (IN PT license). Current BLS/CPR certification. Key Duties Perform patient assessments. Create and modify treatment plans. Provide therapeutic interventions. Document progress accurately.
