RN Clinical Supervisor

Job Description Safe Life Home Health is seeking an office based Clinical Case Manager to support referral intake, case triage, coordination of care, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an RN, LPN, or LVN who is organized, responsive, and comfortable managing a high volume of incoming cases in a fast paced home health setting. We now serve patients through two branches: Lombard branch with a 4.5 stars Medicare Quality Rating Chicago branch with a 5 stars Medicare Quality Rating The Clinical Case Manager will play a key role in reviewing incoming referrals, triaging cases appropriately, helping determine eligibility and service needs, coordinating with intake and clinical staff, and ensuring smooth case flow from referral through admission readiness. Pay Range $37 to $42 per hour Key Responsibilities Review and triage incoming home health referrals in a timely manner Coordinate with referral sources, hospitals, physicians, discharge planners, and internal staff to gather required documentation Assess referral appropriateness based on clinical needs, payer requirements, service area, and staffing availability Communicate with patients and families regarding next steps, services, and admission process Support case assignment and coordination between intake, scheduling, field clinicians, and branch leadership Track pending referrals and follow up on missing documents, physician orders, insurance information, and clinical records Help prioritize urgent and high acuity cases for timely response Collaborate with the Director of Nursing, Intake team, and branch staff to support efficient case management workflow Maintain accurate and timely documentation in the EMR Assist in resolving referral and admission barriers to improve conversion and patient experience Ensure compliance with company policies and applicable home health regulations Support both Lombard and Chicago branch operations as needed Qualifications Active RN license in good standing Prior experience in home health, intake, referral management, case coordination, or clinical triage preferred Strong understanding of home health services, documentation, and care coordination processes Excellent communication, organizational, and multitasking skills Ability to work efficiently in an office-based, team-oriented environment Strong attention to detail and ability to manage multiple cases simultaneously Experience with Kinnser is a plus Preferred Skills Knowledge of Medicare and other payer guidelines related to home health admissions Experience communicating with physicians, hospitals, rehab centers, and referral partners Ability to identify priority cases and elevate appropriately Comfortable using EMR systems and standard office software Work Setting Office-based position Supports operations across Lombard and Chicago branches Advantages-Benefits 401(k) Health insurance Paid time off (2 weeks, 5 casual/sick days, and public holidays) Tuition reimbursement #J-18808-Ljbffr Safe Life Home Health Care, LLC
