Habilitation Specialist 1

Responsibilities Coordinate, develop and provide habilitation services for individuals in an IRA setting. Develop and modify Community Habilitation and Habilitation Residential Plans, Safeguard Summary Plans, Community Inclusion Plans, money management assessments and Personal Expenditure Plans as needed. Coordinate with clinical staff to ensure all individual needs are met and programs are developed/modified as appropriate. Provide training and hands‑on assistance in the proper delivery of daily services. Provide oversight of inclusion experiences by participating in community activities for individuals. Provide training and oversight in the proper documentation of Medicaid billing documents; community habilitation and residential habilitation monthly notes; and EHR notes in relation to the implementation of services. Develop and maintain a good working relationship with care coordinators and staff that serve individuals. Attend life‑plan reviews for all individuals on your caseload, work collaboratively with care coordinators and update community habilitation and residential habilitation plans and safeguard summary plans that support the life plan. Develop and maintain a good working relationship with IRA managers and house staff to ensure implementation of community habilitation and residential habilitation plans and that staff resources are available for the needs of the individuals. Effectively and consistently communicate with IRA managers, house staff and other team members. Be responsible for content and organization for the individual program charts and house filing system in relation to habilitation services. Participate in annual chart audits. Assist with other habilitation service needs as warranted. Function as a liaison for individuals in the community. Attend and participate in all mandatory trainings. All other duties as assigned. Minimum Qualifications Competitive: Candidates must be reachable for appointment on the current Civil Service Eligible List for this title. Open Competitive Qualifications: A bachelor’s or higher‑level degree in a human services field (e.g., audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology, therapeutic recreation including art, creative arts, dance, drama, music, or physical education; related disciplines in human behavior or human skills development). For the purposes of these qualifications, a degree in a human service field includes the professional disciplines listed above. Examples of non‑qualifying degrees include criminal justice, political science, liberal arts, and business. Transfer: Candidates must have one year of permanent competitive or 55‑b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
