Overview: We are seeking a compassionate and experienced Registered Nurse – Case Manager to join our Home Health Care team. This role offers flexibility in scheduling while providing the opportunity to make a meaningful impact on patients’ lives. Key Responsibilities Conduct comprehensive assessments and develop individualized care plans for patients in their homes. Coordinate and manage patient care, collaborating with physicians, therapists, and other healthcare professionals. Monitor patient progress, provide education, and adjust care plans as needed. Ensure compliance with all regulatory and clinical standards. Serve as the primary point of contact for patients, families, and caregivers. Qualifications Current Registered Nurse (RN) license in Nebraska. Home health care experience is preferred but not required; new graduates are welcome. Excellent clinical assessment, communication, and organizational skills. Ability to travel within the North region to visit patients’ homes. Flexible schedule and reliable transportation.
