Licensed Clinical Social Worker (LCSW)

Yoakum Community Hospital is seeking a dedicated and highly skilled Social Worker to join our interdisciplinary team. In this role, you will be the cornerstone of patient advocacy and mental health support, providing high-quality care through expert assessment, psychodiagnosis, and treatment planning. For regular full-time and regular part-time, we employees offer a sophisticated total rewards suite designed to support your health, your family, and your future: Responsibilities Conduct psychosocial evaluations Develop individualized treatment plans Provide evidence-based therapy Manage the discharge planning process from admission to completion, utilizing a deep knowledge of community resources Maintain timely and accurate clinical documentation, progress notes, and assessments that align with state/federal regulations Familiarity with trauma-informed approaches and clinically supported treatment methods Current Texas LCSW License required. Experience in a hospital or clinical office setting is highly preferred.
