Lead Registered NurseAssisted Living Program (ALP) / LHCSAPosition OverviewThe Lead RN is responsible for overseeing clinical operations, resident assessments, regulatory compliance, and nursing coordination within the Assisted Living Program (ALP) and Licensed Home Care Services Agency (LHCSA). This role ensures high-quality resident care, accurate documentation, staff oversight, and compliance with Department of Health (DOH) and organizational standards.Key ResponsibilitiesClinical Oversight & Resident CareConduct in-person nursing assessments for new admissions, recertifications, significant change evaluations, and post-hospitalization returnsComplete Start of Care, UAS assessments, reassessments, and care plan developmentAddress urgent and emergent resident clinical needs and coordinate services as neededComplete fall risk, nutritional assessments, and ongoing chart auditsParticipate in DOH surveys, audits, and performance improvement initiativesDocumentation & ComplianceMaintain accurate and timely documentation in HCM, CompuCare, and state systemsUpload and manage required resident documentation, certifications, and plans of careEnsure compliance with ALP/LHCSA regulations, policies, and assessment timelinesConduct quarterly chart audits and annual chart thinningStaff Oversight & EducationOrient aides to residents and individualized Plans of CareConduct aide supervisions, skills competencies, and annual compliance requirementsProvide clinical education and re-education to caregiving staff as neededReview and sign off on duty sheets and care deliveryCoordination & CommunicationAttend required nursing meetings (daily, weekly, monthly, quarterly as applicable)Submit weekly Lead RN reports to leadershipCollaborate with interdisciplinary teams, case managers, providers, and leadershipManage HealtheConnections access, consents, and medical record retrievalRequired QualificationsActive Registered Nurse (RN) license in good standingExperience in assisted living, home care, long-term care, or related clinical settingsStrong knowledge of resident assessments, care planning, and regulatory complianceProficiency with electronic health records and documentation systemsExcellent organizational, communication, and leadership skillsPreferred ExperienceALP and/or LHCSA experienceFamiliarity with UAS assessments, HCM, CompuCare, and DOH requirementsExperience supervising and educating nursing and aide staffWork ScheduleFull-timeMix of in-person assessments, administrative work, and meetingsWhy Join UsLeadership-focused nursing roleMeaningful impact on resident care and clinical qualityCollaborative clinical environmentOpportunity to influence care standards and staff developmentCompetitive salary, great atmosphere and the opportunity to work in a nurturing environment where patient care is the number one priority.Salaried position with benefits and paid time off.40 hours per weekSalary commensurate with experience.$80,000 - $100,000
