Department Overview

This position includes weekend shifts at an enhanced pay rate to manage call duties and assigned home visits.

Job Summary

The Registered Nurse (RN) will employ the nursing process—comprising assessment, diagnosis, planning, implementation, and evaluation—to deliver professional nursing care to patients in their homes. This role involves collaboration with various healthcare professionals, including physicians, to effectively plan, execute, and assess patient care. The RN will demonstrate proficiency and operate within the complete scope of nursing knowledge, adhering to age and population-specific standards relevant to their clinical environment.

The RN will provide care across all stages of preventive health, maintenance, diagnosis, treatment, and follow-up as patients progress through their healthcare journey. Responsibilities include:

• Planning and coordinating patient care
• Conducting patient assessments
• Providing patient education
• Triage and various nursing interventions

This role must comply with the standards of nursing practice as defined by the Nurse Practice Act and relevant Administrative Rules in the state of practice. Our organization acknowledges the Scope and Standards of Practice and the Code of Ethics for Nurses as established by the American Nurses Association, which serve as the foundation for nursing care and professional behavior.

Qualifications

• Graduation from an accredited nursing program is required, including institutions recognized by the American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). A Bachelor's Degree in nursing is preferred.
• A minimum of one year of professional nursing experience is preferred.
• Must possess an unencumbered RN license from the State Board of Nursing in the state of practice, or hold multistate licensure if in a Nurse Licensure Compact (NLC) state. Required department-specific competencies and certifications must be obtained and maintained. A valid driver's license may be necessary depending on the location.
• Basic Life Support (BLS) certification is required. Certification as an Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) is preferred.
