You will listen, observe, and document findings to develop care plans that truly reflect the patients story./liliCare Planning: Implement and document personalized care plans to ensure quality and continuity. You will make necessary revisions based on patient needs and outcomes, ensuring care evolves as the journey progresses./liliEducation: Be a trusted consultant and educator for patients and families. Explain complex disease processes, teach self-care techniques, guide end-of-life care, and address dietary needs. You will also provide training for other staff as needed./liliCare Coordination: Determine the scope and frequency of services needed based on the patients condition. You will coordinate seamlessly with our interdisciplinary team to prioritize care needs and ensure everyone is on the same page./liliPreventive Care: Initiate appropriate preventive and rehabilitative nursing procedures, applying strict infection control practices to prevent contamination and transmission of disease./liliClinical Documentation: Prepare clinical and progress notes that reflect progress toward established goals. You will ensure compliance with third-party payor requirements while maintaining the highest standards of patient care documentation./liliCommunication: Maintain effective, empathetic communication with patients, families, team members, and other healthcare professionals.
