Ardor Health Solutions is seeking a dedicated Physical Therapist for a Home Health assignment with CenterWell Home Health, a Humana company, serving patients throughout the St. Louis metropolitan area. This position offers the opportunity to work independently while helping patients regain mobility, improve safety, and maintain independence in their homes.

Responsibilities

• Complete home health evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide gait training, therapeutic exercise, balance training, and fall prevention education
• Complete OASIS documentation when applicable
• Collaborate with physicians, nurses, case managers, and caregivers
• Educate patients and families on disease management and home safety
• Maintain timely and accurate documentation

Requirements

• Active Missouri Physical Therapist License
• Graduate of an accredited Physical Therapy program
• Current BLS Certification
• Home Health experience preferred
• OASIS experience preferred
• HomeCare HomeBase experience preferred
• Valid driver's license and reliable transportation
