Title Advanced Practice Provider (APP) - Nurse Practitioner (Travel) Job Summary The Advanced Practice Provider (APP) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN). The APP ensures good communication, efficient and patient‑centered operations, and effective service delivery for every patient. Responsibilities include coordinating and navigating the care for all patients referred to UBHN, from primary care to ancillary services and specialties, and maintaining compliance with business laws and ethical standards. This position requires travel across the state of South Carolina. Education and Experience Requires a bachelor’s degree in a job‑related field and 3 or more years of job‑related experience, which may be substituted by an equivalent combination of certification, training, education, and/or experience. Certifications and Licensure Current licensure as Nurse Practitioner with SC LLR, in good standing. Preferred Qualifications At least one year of experience working in a physician practice. Experience coordinating care of patients across different areas or services of a health system. Knowledge, Skills, and Abilities Ability to communicate effectively. Ability to assess, identify, and address individual needs. Knowledge of, or the ability to learn, medical terminology; use of medical equipment and following clinical orders. Primary Responsibilities Serve as the primary point of contact for all primary care patient referrals within the UBHN, taking calls from providers/offices and connecting with patients. (30%) Work with referred patients to ensure that all clinical services are received timely and appropriately, and that results are communicated within the UBHN following established protocols. (30%) Follow up with patients to provide appointment reminders and ensure patient comprehension and compliance. (20%) Work collaboratively with assigned physician(s) to regularly discuss care and management of assigned patients. (15%) Accept and perform other related duties as assigned. (5%) Employment Conditions Full‑time, 40 hours per week; standard schedule 8:30 a.m.–5:00 p.m., with flexibility required to meet departmental needs. This is a staff time‑limited (STL) position for 12 months.
