Licensed Vocational Nurse (LVN) / Licensed Practical Nurse Specialty: Home Health Duration: 13 Weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 Experience: Minimum 1 Year Acute Nursing Care Required, Home Health Experience Required License: Massachusetts LPN License Certifications: Active American Heart Association BLS or American Red Cross Certification Must-Have: - AGENCY Massachusetts Temporary Nursing Service Agency License (Nursing Pool) - Ability to exercise independent judgment - Demonstrated interpersonal skills - Ability to maintain confidential information - Willingness to accept on-call responsibilities - Current Driver License and Insurance - Ability to drive an automobile in inclement weather - Flexibility regarding work hours

Description: The Community Health LPN provides nursing care of the sick and injured at home. The LPN intervenes to promote, maintain, or restore health to prevent illness and effect rehabilitation. The LPN collaborates with other healthcare providers, professionals, and community representatives to coordinate care at home. The LPN evaluates the individual's and family's response to interventions. The LPN must function independently under general guidance and follow established guidelines and protocols. Duties require independent action and judgment. Utilization of time management, communication, planning, delegating, and problem-solving skills is essential in providing care. Critical thinking skills are necessary. The LPN must maintain emotional stability in emergency settings. History of dependability in meeting the demands of a committed work schedule is essential. Requested time off during the course of this assignment must be noted prior to submission. Travelers may not take all holidays off. The LPN must be willing to work during holiday blocks as needed. Experience with self-scheduling of caseload is preferred. A working knowledge of home care reimbursement and regulatory guidelines is required. The traveler must have their own vehicle for home care provision.
